Rak płuca to nowotwór złośliwy, który rozwija się z komórek wyściełających oskrzela i tkankę płucną. W Polsce rozpoznaje się go co roku u około 21,5 tys. osób, a z jego powodu umiera około 22 tys. osób rocznie. To najczęstsza przyczyna zgonów z powodu nowotworów w naszym kraju (dane Krajowego Rejestru Nowotworów za 2022 rok).
Lekarze wyróżniają dwa główne rodzaje raka płuca. Większość zachorowań to rak niedrobnokomórkowy (NSCLC), do którego należą między innymi gruczolakorak i rak płaskonabłonkowy. Rzadszy jest rak drobnokomórkowy (SCLC), który zwykle rośnie szybciej i wymaga innego leczenia.
W raku niedrobnokomórkowym o wyborze leczenia coraz częściej decyduje nie tylko stopień zaawansowania, ale też cechy samego guza. W próbce z biopsji oznacza się zmiany w genach, takich jak EGFR, ALK, ROS1 czy KRAS, oraz poziom białka PD-L1. Od tych wyników zależy, czy pacjent może otrzymać leczenie ukierunkowane molekularnie lub immunoterapię. Zależy od nich również to, do których badań klinicznych może się zakwalifikować, bo wiele badań dotyczy wyłącznie osób z konkretną zmianą genetyczną.
Leczenie ustala zespół lekarzy na podstawie typu raka, jego zaawansowania i stanu zdrowia pacjenta. We wczesnych stadiach podstawą jest często operacja, czasem uzupełniona leczeniem przed nią lub po niej. W stadiach bardziej zaawansowanych stosuje się radioterapię, chemioterapię, immunoterapię i leki ukierunkowane molekularnie, często w połączeniach.
